Where: USA Youth Sports Complex in Appleton, Wisconsin
When: June 6-8, 2008
Divisions:
Co-Ed:  U-7  U-8  U-9  U-10  U-12  U-14  U-16  U-19
Girls:  U-7  U-8  U-9  U-10  U-12  U-14
(Divisions may be combined when necessary)
Entry Fees: $150 for Non-ASC Teams - Please make checks payable to the Appleton Soccer Club
Number of Games: 3 Guaranteed
Awards: Individual participation awards to all players and coaches. Individual trophies to first and second place teams of U12 and older age groups.
Rosters: Rostered players only - no guest or replacement players are allowed.

Tournament Application Form: All forms are listed in the link on the left.
  • Print the Application Form, fill it out, and mail the application form with a check for $150 payable to ASC.
Additional Tournament Form for All Teams:
  • Player Waiver Form (Required at Check-in) - Each player on the team must complete this form and acquire a parent sigature to be eligible to play in the tournament.
Additional Tournament Forms for Non-Wisconsin Teams:
  • Tournament Sanction Form: This form is used to help you complete the "Permission to Travel Form".
  • USSF Permission to Travel Form: (Required at check-in) - If your team is traveling to Wisconsin from out of state, please begin the process of acquiring permission to travel from your State Association. In some cases, it can take more than 30 days to acquire.
