Managers Checklist

1.   Complete Application and submit with the required fee by April 5th, 2008.

2.   Complete Hotel Registrations if staying overnight

3.   TEAM CHECK-IN:  Please bring the following items with you to the Team Registration Tent at a minimum of at least 1hr. before your first game:

  • Player Passes
  • Official Copy of Team Roster
  • Liability Waiver Form for Each Player
  • Guest Player Form if Applicable
  • Permission to Travel form required for out-of-state teams only.  Obtain this form from your state association.
