Print, fill out and mail your tournament application form along with your check for $375.00 payable to:  Appleton Soccer Club.  Along with the application please include other information (such as teams in your league,  teams played in tournaments and results of games played) that will be helpful for Selecting, Bracketing and Seeding teams for the tournament.

Player Waiver Form: (Required at check-in) - Each player on the team must complete this form and acquire a parent/guardian signature to be eligible to play in the tournament.

Guest Player Form: (Required at check-in) - If your team is using guest players there is a maximum of 3 guest players per team.
